Reminder–How to Remove Your Registration Hold

Important Message About Your Academic Status

Dear Student,

We want to remind you that a registration hold was placed on your student account earlier this month, as a result of being on academic or progress probation. This means you won’t be able to register for summer or fall classes, order transcripts or receive any certificates or degrees until the hold is lifted.

This is an opportunity for you to get back on track. The Academic Awareness Program has counselors and resources to support you in improving your academic performance. Here are the steps you must follow to have the probation hold lifted.

First: Complete the Student Self-Assessment and Action Plan (if you haven't already done this), before you meet with your counselor.

Second: Meet with Phong Lam, your counselor on the AAP team, as soon as possible. Phong will assist you with reviewing the self-assessment, setting goals and removing your registration hold. If you haven't already met with Phong this quarter:

Third: Follow up with Sara Partida Medina, your student success specialist. ara will check in with you to offer support, answer questions, schedule counseling appointments and provide referrals for other services. You may reach Sara:

You must follow these steps in order to have your registration hold removed.

You can learn more about academic and progress probation on the Probation Standards and Requirements webpage. If you have any questions, please don't hesitate to contact your Academic Awareness Program team.

Best wishes,
De Anza College
